Fill out our online form to request an appointment. If you are having a dental emergency, please call us right away.

Fill out forms
If you are a new patient, speed up the process at reception by filling out your health history form ahead of time. Feel free to email it to us [ cornwalldentaloffice@gmail.com ] or bring it to your first visit.

Prepare your insurance information
Email us a photo of the front and back of your insurance card, or bring it to your first visit. Please be familiar with what percentage your insurance covers for basic, routine (hygiene) and major dental treatment, as well as your annual maximum, and if your insurance takes a deductible.

No insurance?
No problem. We accept all major credit cards, interac, and cash payments for our services. If you have children under the age of 18, and they are not covered by an insurance plan, they may qualify for the provincial CDCP (children's dental care program). Patients covered by social services must arrange approval with their representative prior to their appointment date.
